Unite Us, a technology platform, facilitates closed-loop referrals and care coordination for clients across different organizations. The Unite Us Network, live in 19 Utah counties, has over 200 partners using the platform to connect their clients to resources and provide coordinated care. Network providers across sectors can send and receive secure electronic referrals, track every person’s total health journey, and report on tangible outcomes across a full range of services in a centralized, cohesive, and collaborative ecosystem.

You’ll hear from a Super-User Panel about how this platform helps them support their clients and meet the goals of their organization. The panel will share their best practices, learning curves, and why they have been so successful.

About Our Speaker 
Katie Prawitt, Senior Community Engagement Manager

Unite Us is a technology platform, free of cost for nonprofits, that connects providers together to facilitate closed-loop referrals and care coordination for their clients. The Unite Us Network in Utah is live in 19 counties across the state with over 220 partners using Unite Us to support their clients and organizational goals.

Katie works to support nonprofits and community-based organizations in all stages of utilizing the Unite Us platform, from demoing the platform, to ensuring organizations have a solid workflow to adopt the platform, she and her team are there to support organizations through the entire process! She works to ensure Unite Us is a robust and usable tool for all organizations who join.

Katie has a background in community coalitions and public safety working for Salt Lake County Health Department and South Jordan Police. She holds a master's degree in Community Leadership and bachelor's in public health.
